Requests for CERF/OM  Eye Clinic reimbursement should be made to the CTCA Foundation

1.  Include the date and location of the CERF/OM clinic and the name of the participating affiliate CTCA Cairn Club

2. State the number of Cairns examined and the overall findings from the veterinary opthalmologist (i.e 22 Cairns examined.  4 had cataracts and 1 had early signs of OM).  Do not include any names of dogs or kennel names - just
need numbers and the conditions that were seen

3. How much is being requested for reimbursement (up to $400 per clinic)

4. How the check should be made out and where to send it

5. Send the email for reimbursement to Diane Ondo at Melcairns@verizon.net and also copy Pam McDonald, Foundation Chair at maidstonepgm@aol.com  Please also send a copy of Eye Clinic results to Linda Heiner, HRCC Eye Coordinator at L2heiner@cox.net .
